Please follow these few simple steps when shipping to ACT Services:
      - Use appropriate corrugated shipping carton and bubble wrap packaging.
      - Securely wrap each unit separately in bubble-wrap.
      - Print out and enclose the completed service work request below to include your
        Purchase Order or Repair Authorization # with the device description and
        Serial Number for each unit shipped.
