Original OEM parts are used. All maintenance and repairs are done by trained biomedical technicians.

To initiate depot repair service for overhauls, scheduled maintenance or repair to meet compliance standards complete the online form. For shipping use appropriate corrugated shipping carton and bubble wrap packaging. Ship units to ACT Services, 1428 Pacific Street, St. Paul, MN 55106-6575.

Please follow these few simple steps when shipping to ACT Services:

• Use appropriate corrugated shipping carton and bubble wrap packaging.
• Securely wrap each unit separately in bubble-wrap.
• Print out and enclose the completed service work order below to include your Purchase Order or Repair Authorization # with the device description and Serial Number for each unit shipped to ACT Services within a securely sealed shipping carton.
• Show Departments or Individuals to whom units are to be returned to following service, together with a full and complete return address to assure prompt and accurate delivery.
• A telephone number with extension # will be appreciated as well.
